Quality Transfers of Care in the West Midlands – Self-Assessment Toolkit

The West Midlands Integration Leadership Group (ILG) exists to support joint improvements between, health, social care, adults and children’s services in the West Midlands.  The group has recently agreed a number of actions aimed at addressing delayed transfers of care & continuing health care challenges in the West Midlands.

Local System Resilience Groups are asked to support this project by completing a self-assessment questionnaire that is aimed at identifying good practice and opportunities for improvement.  We anticipate that the outcome from this work will support a common definition of DToCs and support the development of improved processes, systems and expectations across the West Midlands.

The questionnaire, which is based on the Helping People Home team’s DToC management questionnaire, can be completed in a number of ways:

  • Facilitated discussion between Project Team and Council, Acute Trust and CCG representatives
  • Self-assessment, led by SRG’s nominated representative
  • Telephone conference call between Project Team and Council, Acute Trust and CCG representatives
  • Telephone interview between Project Team and SRG’s nominated representative

Completed questionnaires are required no later than 30th June 2015. We’d like SRG partners to agree the answers before submission.

The Project Team will present its analysis of the completed questionnaires at a joint event for HWB partners to share the findings and agree actions. This should allow  time for local systems to make any required changes before the onset of winter pressures later in 2015.
